Are you a Nursing Professional looking for Independent Contractor assignments? Below you will find all the necessary forms to fill out for Independent Contractor opportunities. These opportunities are for nursing (RN,LPN,CNA) contracts only.
- Information Sheet - Fill out an Information Sheet online, and have it sent directly to
our Contract Administration Team!
- After completing the Information Sheet, please call the office at 215-887-8050 to make an appointment for an in-office visit.
- When visiting the office, please bring your Nursing license, CPR, 2-Step PPD/Chest X-ray report/QuantiFERON Report, Drivers License and Social Security card, Business Card, ID card from agencies you have worked for and your business registration information (optional).
